Please read the Client Agreement, Notice of Privacy Practices, Social Media Policy, and Tele-Therapy Policy. Then fill out the Client Information Form, either electronically or by hand, and scan it in if necessary. I can show you how to transfer the file to me securely during our first Zoom session. Otherwise you can send a copy through the US Mail to my office.
The Client Information Form is the only one that I need back from you. For couples, each of you must fill out and sign separate forms. Minor children must fill out the Minor Information Form which must be signed by both parents. Please print copies for your files.
